Your first visit to Brownsboro Dermatology involves a few special steps so that we can get to know you. To understand what to expect, please read through this page. You'll find all the practical information you need, such as a map and directions to our office, practice hours, payment policies and more. You can also save some time by printing out and completing the patient forms in advance of your appointment.

Returning Patients

In order to ensure that our information is correct, we ask that all patients provide us with updated, online forms once each year. You may use the forms on this page.

Patient Forms

Our patient Demographic Form and History Form may be completed online just by clicking on the link below and downloading the form to your computer. You can then complete, print the forms, and bring them with you to your visit. Or, you may print the forms, complete them, and bring them with you. We do not accept email versions of this form. Please note that there are two History Forms: one for our patients 18 and over and one for patients 17 and under.

Map and Directions

We are located on the 2nd floor of the Rodes Building, 4938 Brownsboro Road, Suite 206. This is just west of Herr Lane, beside the McDonald's. Holiday Manor Shopping Center is across Brownsboro Road to the north.

What To Expect

Being well-prepared for your appointment will ensure that the physician has all of the needed information including a list of your medications to provide the best possible care for you. Please bring your insurance card, a photo ID, and referral, if required by your insurance. Female patients, please remove facial foundation prior to your visit to ensure optimal skin examination. Also, take some time to review our staff page and familiarize yourself with the physicians. It will also help alleviate any unnecessary anxiety you may be feeling prior to your first appointment.
